Dog Daycare





Registration Requirements:
	Must be current on all vaccinations and provide veterinarian records for DHLPP, Bordatella and Rabies.
	Must be in good health. Owners must certify that their dog(s) are in good health and have not been ill with a communicable condition in the last 30 days. Dogs who have been ill with such a condition in the last 30 days will require a certification of health from a veterinarian to attend daycare.
	Must be non-aggressive; food or toy protective. All dogs will be evaluated by a Happy Puppy staff person for aggression during registration. Owners will also certify their dog has not harmed or shown aggression towards any person or dog.
	Must be on flea control and Heartworm preventative.
	Must be spayed or neutered if over 7 months of age.
	Must sign and complete an application and owner agreement.

Registration/Evaluation Fee:
There is a $20, non-refundable registration/evaluation fee for each dog.

Please Remember...
...that the rules of registration are for the safety and protection of all dogs who attend our daycare. Please respect these rules and be mindful of your dog's health condition. If your dog exhibits signs of a communicable illness, he/she will be quarantined and you will be asked to pick him/her up immediately.
